Food portions?

That's a start but the physiological response to bread (gluten) is somewhat mimiced when rice, corn, soy, and even dairy are consumed. Your advice can work for a lot of folks but this person sounds far beyond that stage. To really "kick" sugar you're talking dropping out carrots and a whole bunch of other stuff most people don't consider. That's why keto is more likely required here.

You have to cut it ALL for the first few weeks.
Get into ketosis first. Stay in ketosis for a few weeks. You'll lose the bloat, lose the appetite, start dropping some lbs, get into an exercise routine - Then start incorporating leafy greens and later the carrots and other veggies with higher carbs counts...
 
A buddy who retired from work had a great story along these lines. He was a Nam vet who had flown attack choppers. He held PT records at Parris Island for years and is still a big, fit, man.
He and some buddies hit an all you can eat Chinese Buffet one night (or many nights) and were doing some serious damage. Apparently the owner started looking out the swinging doors from the kitchen and shaking his head. After this went on for awhile, and the boys kept hitting the buffet, he finally came stalking out of the kitchen and pointed to the door.
“You go, you no pay, you just go! Go now, you no pay, just go!”

In the past, I would look for sugar all over the place. Still have to admit, I love and crave any sugar I can get my lips or mouth on. I've often felt getting a sugar rise the first thing in the morning and virtually all times throughout the day or night. It really helped sustain my energy and stamina.

Wow. Do you consider yourself physically fit? Height proportionate to weight?

I think I'm overall becoming more fit, but I do have some stubborn love handles I need to work on this summer. Height to weight? I consider stamina proportionate to length of my workout regimen. Building extra length of my workouts helps me to build endurance the most. That makes for a rather satisfying work out experience, time and again. And, yes, my love for sugar can only be quenched in certain ways. Its kind of a burden I've learned to live with.
